Remember to Get Your Child Vaccinated

Mailout of preliminary checkup sheets for FY2026

Shinjuku City will mail out preliminary checkup sheets to all eligible residents at the appropriate time for each vaccination. Please bring the correct preliminary checkup sheet and have your child vaccinated at a designated medical institution in the city (appointment required).

If you move to Shinjuku City after the mailing period

If you move to Shinjuku City after the preliminary checkup sheets have already been sent out, you may miss some vaccination information. If there are any vaccinations your child has not yet received, please call the Public Health Promotion and Disease Prevention Division (03-5273-3859) or visit the Shinjuku City website.

Children of an age equivalent to the final year of kindergarten, nursery school, kodomoen, etc.

Children of this age are eligible for the second routine dose of the MR (measles-rubella combination) vaccine. The preliminary checkup sheets for this were mailed in late March.
