Note: If information must be confirmed with the municipal office administering your previous address, your notification may not be processed. Notifications of moving into Shinjuku City from overseas cannot be processed.

|National Health Insurance||Procedures related to obtaining eligibility
Note: A certificate of loss of eligibility of social insurance [employee’s insurance] is required. If you do not have any dependents, a certificate of retirement can be used instead. If you come to the service counter with one of the single documents sufficient to confirm ID noted in “Documentation needed to confirm personal identification” in the precautionary note below 1, you can receive your health insurance card on the same day.

Note: The health insurance certificate of the applicant’s place of employment is required. Foreign residents must also bring their residence card or special permanent resident certificate (or foreign resident registration card for those who have not yet converted their card to a residence card) and their passport.

In some cases, if you do not sufficient documentation to confirm your personal identification, applications for notifications and certificates cannot be processed. Please be sure to call in advance to confirm details.
*1: Always be sure to bring documentation to confirm personal identification when you come to the service counter. If a proxy completes the procedures on your behalf, the proxy must have a proxy letter as well as documentation to confirm their personal identification.
Documentation Needed to Confirm Personal Identification
- Single documents sufficient to confirm ID: A valid driver’s license, passport, basic resident registration card (with photo), Handbook for the Physically Disabled, residence card, special permanent resident certificate, or foreign resident registration card
- When two documents are required to confirm ID: A valid health insurance card, Medical Care System for the Older Senior Citizen health insurance card, Long-term Care Insurance certificate, pension notebook, basic resident registration card (without photo), or student ID (with photo)
*2: Those who can apply are restricted to the applicant himself/herself or a family member living in the same household for a copy of the residence records; or a person listed in the family register or a spouse or blood relative of such (please bring documentation verifying the family relationship) for a copy of the family registry.
*3: For registration to be completed on the same day, the application must be filed by the applicant himself/herself in person, who must present one of the single documents sufficient to confirm ID noted in “Documentation needed to confirm personal identification” noted above.
*4: Please bring your personal seal registration card when applying for a personal seal registration certificate.
*5: If you come to the service counter to file a notification of marriage, divorce by consent, adoption, severing of ties by mutual agreement, or acknowledgement of paternal ties, or to request (withdrawal) nonacceptance of such notifications, please bring ID document(s) as listed in “Documentation needed to confirm personal identification” above.
