Income Tax Reports and Resident Tax Reports/You Must State Individual Numbers (“My Number”) on Reports for 2016 Onward

Starting with reports for 2016, you will be required to write your individual number and those of your dependents on reports related to income tax, special income tax for reconstruction, consumption tax and local consumption tax, gift tax and resident tax. If you file a report in person at a service counter, please bring documents to confirm your identity (see below). If you file by mail, please attach a copy of the document confirming your identity.

Examples of Documents to Confirm Personal Identity
Documents to confirm your individual number and your personal identity are required. Please show either 1 or 2 below.
1. Your individual number card (for individual number confirmation and confirmation of personal identity)
2. Two items are required: A copy of your resident registration slip with your My Number written on it or a notification card (for individual number confirmation) and a driver’s license, etc. (for confirmation of personal identity)
* A valid notification card that you can use to confirm your individual number is one that requires no change in name or address, or one for which you have already completed change procedures.
Note: If you file your income tax report from home using e-Tax, there is no need to show documents or file copies of documents to confirm your identification.
